Eligibility

The applicant paid medical expenses in a single month that exceeded the statutory self-payment limit (自己負担限度額).

Required documents

• High-cost medical expense notification postcard
  高額療養費のお知らせハガキ
• Receipt for the month of medical consultation (not required for those aged 70 and over)
  受診月の領収書（70歳以上の方は不要）
• Identification document
  身分証明書等
• Bank account in the name of the National Health Insurance household head (if transferring to an account not in the household head's name, a power of attorney from the household head is required)
  国保世帯主名義の銀行口座（世帯主名義以外の口座への振込は世帯主からの委任状が必要となります）
