Eligibility

• The applicant must be either the insured person or their dependent at the time of childbirth
• If the applicant has lost insured status, they must have had at least one year of continuous insured period before loss and given birth within six months after loss

Required documents

• Application form for health insurance childbirth lump-sum payment
  健康保険出産育児一時金支給申請書
• Proof that the direct payment system was not used (e.g., receipt or statement showing 'direct payment system not utilized')
  医療機関等から交付される直接支払制度を利用していないことを証明する書類のコピー
• Receipt or statement explicitly stating 'this delivery is covered by the Obstetric Medical Compensation System' (if applicable)
  「産科医療補償制度の対象分娩であること」が明記された領収・明細書のコピー
• Physician or midwife's certification of childbirth (if municipal certification is unavailable)
  出産を担当した医療機関等の医師・助産師の証明書
• Document verifying childbirth (e.g., family register transcript, certificate of registered matters, birth notification acceptance certificate) — for stillbirth, stillbirth certificate
  出産したことを確認できる書類（戸籍謄（抄）本、戸籍記載事項証明書、出生届受理証明書 等）（死産の場合は、死産証書（死胎検案書）等）
• Physician or midwife's certification from overseas medical institution (for overseas childbirth)
  出産を担当した海外の医療機関等の医師・助産師の証明書
• Document proving overseas travel during childbirth (e.g., passport, visa, airline ticket)
  出産した日（期間）において、実際に海外に渡航していた事実が確認できる書類（パスポート、査証（ビザ）、航空チケット等のコピー）
• Consent form permitting Kyosai Kenpo to contact the overseas medical institution
  海外出産の事実、内容について、協会けんぽが当該海外出産を担当した海外の医療機関等に照会することに関する当該海外出産をした者の同意書
• Certified translation of foreign-language documents (signed by translator with address and phone number)
  翻訳文（翻訳文には、翻訳者が署名し住所及び電話番号を明記してください）
• Family register transcript proving relationship to deceased insured person (if applicant is heir)
  被保険者との続柄がわかる「戸籍謄本」等
• Copy of My Number Card (both front and back) or, if unavailable, (i) copy of residence certificate with My Number, and (ii) copy of photo ID (e.g., driver's license, passport)
  本人確認書類
• Family register transcript or other document proving legal agency (for legal agent), plus copy of photo ID
  戸籍謄本、その他法定代理人を証明する書類等
• Power of attorney (free-form) plus copy of photo ID (for authorized agent)
  委任状（任意の様式）
