Who it is for

Enrollee of Naha City's National Health Insurance who has given birth at or after 12 weeks of pregnancy (including stillbirth or miscarriage); benefit is paid to the household head

Eligibility

• Applicant must be enrolled in Naha City's National Health Insurance
• Childbirth (including stillbirth or miscarriage) must occur at or after 12 weeks of pregnancy (85 days)
• Benefit is paid to the household head

Required documents

• Maternity benefit application form
  出産育児一時金支給申請書
• Proof of identity of the person who gave birth (e.g., eligibility confirmation document, My Number card)
  分娩した方の本人確認書類（資格確認書、マイナンバーカード等）
• Receipt and itemized bill for childbirth expenses issued by medical institution
  医療機関等で発行される出産費用の領収・明細書
• Written agreement confirming use or non-use of the direct payment system
  医療機関等で交わす直接支払い制度を利用する旨の合意文書
• Proof of identity of the applicant (e.g., My Number card, driver's license)
  手続きする方の本人確認書類（マイナンバーカード、運転免許証等）
• Bankbook or other document verifying bank account details (account must be in household head's name unless accompanied by power of attorney)
  預金通帳または振込先の確認ができるもの
• Physician's stillbirth certificate (required for stillbirth or miscarriage at or after 12 weeks)
  医師の死産証明書
• Official birth certificate or equivalent issued by local public authority or medical institution abroad (for overseas births)
  現地の公的機関や医療機関が発行する出産の公的証明書（出生証明書等）
• Japanese translation of all foreign-issued documents (must include translator's address, name, and phone number)
  現地の公的機関や医療機関が発行する出産の公的証明書の日本語訳
• Passport showing entry/exit dates (or official entry/exit record)
  出産した方の出入国日がわかるパスポート（出入（帰）国記録または旅券でも対応可）
• Consent form authorizing inquiry to overseas public authorities or medical institutions (for overseas births)
  海外の公的機関・医療機関等に対して照会を行うことの同意書
• Local birth registration or family register transcript (if no Japanese birth notification is filed)
  現地の出生届または戸籍謄（抄）本

Deadline

Within 2 years from the day after childbirth
